Once a form is created in Formatta Designer, it can be uploaded to Formatta E-Forms Manager using the New Form screen. In addition, Form Summary and Contact Information can be reviewed and edited so that end users have up-to-date Form Summary Information once the form is published.
To upload revised versions of existing forms, use the Version History screen (part of Form Details).
To navigate to the New Form screen where you can begin to upload a new form, select New Form from the Catalog tab.
Forms created using Formatta Designer can be uploaded to Formatta E-Forms Manager using the New Form screen. Enter the information identified below:
Path to Form: A required field allowing you to browse your local computer to upload a new Formatta form to Formatta E-Forms Manager.
Form Lock Password: Every form uploaded to Formatta E-Forms Manager must be locked in Formatta Designer using a form lock password.
Version Notes: Enter any descriptive text which end users may find useful relating to this form.
All of the above items are required. When complete, press Upload to upload a new form to Formatta E-Forms Manager, and proceed to Step 2 below.
Form Summary information allows you to provide meta data associated with the form. This information is recorded with the form and made available to end users when the form is published. When viewing a form in Formatta Filler, end users can select the Info button to display the information entered here, for both Summary and Contact Information.
Summary Information consists of the following form meta data:
Form Title: Enter a unique name for the form.
Form Category: Select a form category from the drop down list. These categories are pre-defined by a Form Administrator in the Category screen.
Description: Enter a useful and brief description of the form and how it is used. This information will be published with the form.
Number: Enter a number for your form using a numbering classification system. For example, "SF 71" or "GSA Form 1334."
Subject: Enter a form subject associated with the form.
Version: Enter a version, such as 1.0 or 2.1.
Version Date: Enter the date this version of the form was created.
Approval: If applicable, enter the organization who approved the form. Examples include OMB, or Human Resources.
Expiration Date: If known, enter the date when the form is scheduled to expire.
Required fields include Form Title, Form Category, and Description.
Contact Information consists of the following form meta data:
Designer: The name of the form author (the person who created the form in Formatta Designer).
Designer Email: The email address of the form author.
Company: The name of the company or government agency sponsoring the form.
Department: The organization in the company or government agency responsible for the form.
Contact: The name of a contact person or help desk in case end users have questions about the form.
Phone: The phone number used by end users in case they have questions on the form.
Contact Email: The email address for technical support on the form.
Web Site: The web site where users can go for frequently asked questions (FAQ) and other self-service support.
Press Save Changes once you're satisfied with the information. The Form Details screen then displays, allowing you to edit Form Management Properties for your new form.
Once a form is published, Form Summary Information associated with each form can be viewed by end users using Formatta Filler just by clicking the Info button on the Filler toolbar. This meta data allows end users to determine the origin of a form (an important security measure), expiration, version information and date, approval, the form publication URL, and contact information in case they have questions - whether or not they're connected to the Internet.

Forms are password protected in Formatta Designer using Form Locking. Enter the form lock password used in Formatta Designer when uploading a new form to Formatta E-Forms Manager. As a security precaution, every form must have a form lock password specified in Formatta Designer before uploading to Formatta E-Forms Manager. |
