Forms have a life cycle based on a business process and data collection requirements. The first phase of this form publishing life cycle begins with staged forms, proceeds through approval and publication and finally, when the form is no longer needed, expiration. As a form progresses through each phase, Formatta E-Forms Manager graphically displays its status in a progress bar.
After a form is Staged, it must be reviewed and approved by a Level 2 or above Form Administrator. When a form is Approved, it means the form is ready to be scheduled for publication. Forms cannot be re-staged (i.e., moved back to the Staged phase) once they have been approved.
To navigate to the Approved Forms screen, select Approved from the Catalog tab.
The Approved Forms screen lists versions of forms that are ready for review, editing, and approval. Forms must be approved before they can be scheduled for publication.
Click the Formatta Filler icon to view the form in Formatta Filler. Other information includes:
Form Title - The name of the form assigned when the form was initially uploaded using the New Form screen.
Version - The version number assigned by the Form Administrator when the form was initially uploaded.
Category - The category assigned to the form when it was initially uploaded to Formatta E-Forms Manager.
Publish On - The date the form is scheduled to be published, if any. Even if an approved form has a "Publish On" date, it will not be made available until published by a Level 3 or 4 Form Administrator.
Edit - Allows you to edit Form Management Properties associated with the selected version of the form.
Delete - Allows you to completely delete the form from Formatta E-Forms Manager.
The Form Title, Version, and Category information can be changed using the Form Details screen.
To view form details on any Approved form, click the highlighted row when moving your mouse over the Approved Forms screen.
To change the status of a form and update Form Management Properties, select Edit corresponding to the form you want to edit.
Click the Delete icon to delete the form from Formatta E-Forms Manager. A window will display requesting you to confirm deletion of this version of the form and all submissions based on this form.
Keep in mind that ALL forms that were previously submitted or signed by end users will also be deleted. Once a form has been deleted it cannot be recovered.
A Word of Caution - Each version of a form in Formatta E-Forms Manager is a type of "master" record. Any time an end user signs or submits a form, Formatta E-Forms Manager compares its properties (hash, layout, XML, etc.) against those of the "master" form. This process is an integral security measure and guarantees that forms are authentic; however, if you delete this master form, all associated records (i.e., forms signed and submitted by end users) are also deleted.
After approval, forms must be published by a Level 3 or higher Form Administrator. In addition, form versions must have a scheduled publication date, specified in the General tab under Form Management Properties.
Select Edit corresponding to the form you want to publish. The selected approved form then displays in the Form Publishing Information screen. For more information on publishing forms, see the section entitled, "Published."
