Sample Form Library

To help jump-start your e-forms library, Formatta has developed a set of customizable e-forms, included in the Formatta E-Forms Manager installation (under \My Documents\My Forms\E-Forms Manager Sample Forms).  All you need to do to make them your own is open each form in Formatta Designer and change the Company Name text object to your company name, and the Company Address text object to your company address.  Each of the forms below uses the word "formatta" as the form lock password.  You'll need this password to open the forms in Designer.

 

Once customized for your company, upload them to Formatta E-Forms Manager to have a set of ready-made forms.

Meeting Agenda - Use this form to plan a meeting and document the results, including agenda and action items.

Travel Log and Expense Report - Use this form to record travel and expenses.  Includes signing and supervisor review.

Business Debt Tracking Report - Use this Debt Tracking Report form for managing business debts.

Company Letterhead - Use this form for writing internal/external letters.

Daily Planner - Use this form for planning daily tasks.

Disciplinary Notice - Use this form to notify an employee of disciplinary action  to be taken in regards to a specific problem.

Dismissal Notice - A notice given by employers, listing the date and the reasons for the dismissal of an employee.

Employee Checkout Record - Use this form to track items that must be returned or completed upon an employee's termination.

Complaint - Use this form for employees who have a specific complaint.

Employee Consultation - This form records the time frame in which a problem is addressed and the action taken to  correct it by both the employer and employee.

Employee File - Use this form to record pertinent information concerning an employee.

Employee Information Update - Use this form to update employee information, including emergency and medical information.

Prospective Employee Referral - Use this form to provide information about a prospective employee and the current employee who gave the referral.
