Phreesia, Inc. Privacy Policy (revised February, 2011)

At Phreesia, we know you care about your personal privacy and about the terms and conditions that govern your use of this Website (this "Site"). Because we are dedicated to serving your needs and respecting your preferences, we have adopted the policies and practices described in this Privacy Policy and Conditions of Use.

The following statement discloses our standard practice regarding the information we collect and the information we hold on behalf of our customers.

This privacy policy only applies to the Phreesia website. The policy does not pertain to third party web sites that are accessible through Phreesia's website nor does this policy apply to information gathered via our proprietary PhreesiaPads located in various physician offices. Phreesia proposes that you read the privacy policies of any third party site and as set forth on the PhreesiaPads to learn how they collect and use information that is gathered.

Our Collection and Use of Your Personally Identifiable Information

In general, we collect and use information about you for the purpose of contacting you to provide you with information about Phreesia's products and services or to provide you with other information related to Phreesia. We do not sell, rent or otherwise share or disclose your private, personally identifiable information to third parties.

Because of the nature of our business, our services and benefits are not designed to appeal to minors. We do not knowingly attempt to solicit or receive any information from children.

Phreesia specifically collects information from you at several different points, including but not limited to the following:

Information You Provide To Us

On our "Request Information" and "Request a Live Demo" pages, the following information is required: your first and last name, practice name, telephone number, email address, your role within the practice, the practice's specialty, names of physicians in the practice (if role is selected as anything other than Physician), the practice's location, and whether or not the practice has a high speed internet connection. The following information is optional: postal address, number of physicians in the practice, how you heard about Phreesia, your interest in Phreesia, invite code, and secondary contact information (first name, last name, and telephone number). Submission of this information will allow you to find out if you are eligible for Phreesia's products and services, and will help us to better serve you in follow-up communications. This information will only be used for the purpose for which it was collected.

On our "Support" page – Your email address and contact information will be provided to us. This information is only used to respond to your support question or issue.

On our "Contact Us" page - You are directed to send an Email to a specific department within Phreesia. Through this process your Email address and comment or question are collected. This information is only used to respond to your question or suggestion.

Phreesia will treat any information of its visitors to our site as confidential and will not disclose it to any third party without consent.

Correspondence

If you correspond with us via email, the postal service, or other form of communication, we may retain such correspondence and the information contained therein and use such to respond to your inquiry and/or notify you of Phreesia products and services, conferences, news or other Phreesia-related opportunities. Importantly, email and postal address information is never sold, given, or otherwise shared or disclosed to third parties except as set forth herein.

If you no longer wish to receive any promotional communications, you may opt-out of receiving them by following the instructions included in each communication or by emailing us at support@phreesia.com. Updating Personally Identifiable Information

If your personally identifiable information changes, or if you no longer desire our service, you may deactivate it by emailing our Customer Support at support@phreesia.com or by contacting us by telephone (1-888-654-7473) or postal mail (432 Park Ave South, 12th Floor, New York, NY 10016).

Customer Testimonials/Comments/Reviews

We post customer testimonials/comments/reviews on our web site, which may contain personally identifiable information. We do obtain the customer's consent prior to posting the testimonial to post their name along with their testimonial.

URL and IP Address

Like many other Websites, we may collect information about our users' utilization and navigation of our Site. This information helps us to design our Site to better suit our users' needs. We may also use your IP address to help diagnose problems with our server and to administer our Site, analyze trends, track visitor movements, and gather broad demographic information that assists us in identifying visitor preferences.

Cookies

Cookies are pieces of data that a Website transfers to a user's hard drive for record-keeping purposes. The Phreesia site may use cookies to provide enhanced functionality on the site (e.g., user ID and password prompts) and aggregate traffic data (e.g., what pages are the most popular).  These cookies may be delivered in a first-party or third-party context.  Phreesia may also use cookies in association with emails.  While these cookies may be associated with personal information (such as your email address), we do not associate personal information with clickstream data. Our privacy statement does not cover the use of cookies by our tracking utility company. We do not have access or control over these cookies. Our tracking utility company uses session ID cookies to make it easier for you to navigate our site.

This Privacy Policy only addresses the Phreesia's use and disclosure of your personally identifiable information. As this Site may contain links to other Websites, please be aware that we are not responsible for the privacy practices of such other sites, and we are not liable for their misuse of personally identifiable information. As our corporation, services and products change from time to time, this Privacy Policy is expected to change as well.
